Overview
Forms help outfitters and guides review guest information needed for trip readiness in the Mallard Bay mobile app. In mobile, forms are primarily a review workflow: you can see existing forms, check their fields, and review submitted guest answers.

View Forms
Open the mobile Menu and select Forms to open screen_name: Forms.
The Forms screen shows a list of forms for the outfitter. Each form card displays the form name and the date it was added. Tap a form to open its details.
If there are no forms available, the app shows “No forms yet.”
Review Form Details
On screen_name: FormDetails, you can review:
- Form status: Active or Inactive
- Number of form fields
- Date added
- Form fields, including field title, input type, and whether the field is required
- Form submissions, when guests have submitted responses
Submission cards can show the guest name or username, email, submission status, date added, and submitted answers.
Check Booking Readiness
Forms may also appear inside a booking when that booking has related form submissions. In the booking details workflow, the Forms section summarizes how many submissions are completed out of the total required submissions.
Expand the Forms section to review guests and their form submission statuses. When a submission has a form URL, mobile can provide sharing actions such as Share link and QR Code so a guest can access the form from the field.
Create Or Edit Forms
Form creation, editing, form setup, required field configuration, and waiver-style setup are managed in the web dashboard.
From the mobile app, open the Menu and use the web dashboard navigation item for forms or setup work. The dashboard opens from mobile in the in-app browser, so you can still manage form authoring when you are away from a desktop.
